Dear {jury commissioner}:
I'm writing to request that my jury duty date of {date} be postponed. I am at high risk of negative outcomes if I were to contract Covid-19. I understand that the court has mask requirements and other safety measures in place, but after talking with my doctor, we just don't feel that I should take the risk.
Fortunately, it looks like I will be able to get a vaccine by {date}. I would be happy to serve as soon as a month later.
I've attached a note from my doctor, {name}.
I will look forward to receiving notice of postponement and an alternate date.
